When I saw the turgid! film, "Diet Another Day"(which is what I wanted to do after it was over [barf])at the local 15 mini-cini-plex theater, the location scene title at the bottom of the begining of the film was half cutoff, and then the rolling credits at the end were out of focus until they got to midscreen. What causes these problems?

Aperture plate undercut, or picture out of rack, or both.

Gate pressure issue? Damaged lens element? Incorrectly focused anamorph? Very severe rake?
