According to our organist, Bob Gulledge who works for Jordan Kitts, a major distributor for Allen, for which Hazelton was a house organist it came across the company e-mail this afternoon.
He died of an apparent heart attack at home last night. No other info was available.

Very sad day. Tom was one of the regular organists at the Golden State Theatre. He was quite the gentleman and a class act from start to finish. My thoughts and prayers go out to his family and friends.

I was able to hear Mr. Hazelton perform twice at the theatre I used to work (the Bay Theatre, Seal Beach, California). The organists who played there always amazed me with their coordination over four limbs (and four manuals) and ability to replicate the sound of a full symphony, and he was no exception. This news is surprising... he didn't seem very old (or in poor health) at all, perhaps late fifties or early sixties?

Not sure but I believe he was closer to 70, didn't look it though.
