Cause of death at this time is not known. There were certain things you just weren't allowed to show in Japan (they seem to have relaxed a little recently) so when most laserdiscs were made there, a censored version of "Insatiable" was pressed. I haven't seen it but I've heard it's hilarious- lots of cheesy video effects are added to fill in missing time. "Up N' Coming" was the first uncensored porno movie to be issued on laserdisc, but it's still barely watchable because they cut a lot of it for time so that side 2 could be in CAV format. The final scene with John Holmes ends rather abruptly with a video fade.
